Clinical Analysis of Superior Mesenteric Artery Occlusion



Abstract
Superior mesenteric artery occlusion is rare in incidence. It is known that the prognosis after operation is grave when diagnosis and proper treatment is unduly delayed.
A clinical study was performed on 7 cases of superior mesenteric artery occlusion which we experienced at Koryo General Hospital for the past 18 years.
@ES The results were as follows:
@EN 1) The range of age from 51 to 71 years and male to female ratio was 4 : 3.
2) Four cases were admitted through emergency room due to abdominal pain and operated within 24 hours from the symptom onset. While the symptoms were developed during hospitalization in 3cases, and operation was delayed at least 4 days.
3) Heart diseases were a associated in 5 cases. 3cases had myocardial ischemia, and 2 cases had atrial fibrillation.
4) The important clinical manifestations were abdominal pain, abdominal distention, peritoneal irritation signs, vomiting. Characteristically abdominal pain was out of proportion to the physical findings such as tenderness and rebound
tenderness.
Mild
leukocytosis(more than 20,00/§§) was noted in 4 cases.
5) Preoperative impression were superior mesenteric artery occlusion(3 cases), UGI bleeding ( 2 cases; Gastric ulcer bleeding was combined in 1 cases), intestinal obstruction(1 cases) and panperitonitis due to perforation of peptic ulcer(1
cases).
6) operative procedures were segmental resection of jejunum(2cases), resection of ileum and cecum(1 cases), resection of jejunolileum and ascending colon(1 cases) and laparotomy only(3 cases). All cases of bowel resection were survived and all
cases of
laparotomy only were dead. Overall mortality rate was 42.9%.
7) The causes of superior mesenteric artery occlusion were emboli(4 cases) and thrombi(3 cases).
